USE THIS UPLOAD PORTAL TO SUBMIT REQUIRED DOCUMENTS - YOU'LL NEED TO PROVIDE THE FOLLOWING:
1) A copy of your unexpired government issued photo id. Just snap photo or scan (Not over 1000KB or 1MB image)
2) There is a non-refundable fee of $50 for each applicant in order to apply. Click here to authorization the payment of the application fee via credit card. An electronic signature is acceptable.
3) Every applicant must provide verifiable proof of current income. Acceptable proof of income includes the following: *See list below! Normally Just, Government I D, plus First two Items Below are needed.

Acceptable proof of income includes the following:
✔ Paystubs for the last 60 days
✔ Bank statements for the last 2 months showing direct deposit from employer
✔ Court ordered child support or alimony
✔ Most recent W2 if from current employer
✔ If self-employed - Tax returns and last 3 months of personal bank statements
✔ Verification of employment with income completed
✔ Offer letter/contract on letterhead and signed by employer, if starting a new job
✔ Social security or disability income benefits letter
✔ If you are moving from another City, we require verification from your employer that
you will continue with your current employment, or proof of income from your new
employer in the form of an offer letter/contracton letterhead and signed by the employer.
